Sorry for taking so long to answer, I was waiting for this to happen and I can confirm that there is no window behind if this problem happens. This button that is misplaced does follow exlipse from one display to another, meaning, if I move eclipse to left display after some time it will re-appear at different place but it will come at some time.
I've also noticed that behavior in Eclipse. My guess is that some kind of hidden tooltip window/decorator would cause this.
But just like Petar said, I haven't seen this lately either.

Good to know I am not imagining it. Your guess sounds reasonable.
I have few old screen-shots, but don't want to send as problem might be gone with one of newer betas... let's wait and see.
There were a lot of fixes for things like this in the new 3.2 release, I would like to think that it has been solved, but please follow-up if the issue happens again.
Sorry for the long delay, but I've added a feature to the upcoming Beta 12 to help us troubleshoot this issue. It is an Advanced Setting:

Once Beta 12 is released (hopefully this week) just enable this Advanced Setting and the TitleBar Button tooltip will give a ton of useful info for me.
Didn't follow up for a while, but it seems that this issue is specially related to Eclipse (in conjunction with DF probably).
There's one very cool effect: I have enabled the transparency title bar button, so I can actually bring the window to foreground (at 50% opacy).
Since I work with console windows most of the times the revealed windows were console windows, sometimes the Eclipse help window and rarely other ones.
I will further pay attention to Eclipse's activation/foreground state when I again encounter the issue.
Quick follow-up, we fixed a lot of the issues with the TitleBar Buttons in the final version of 3.3.1. After updating, if you're still having issues, please start a new topic.
