I had this behavior with all betas, but only with maximized windows.
Did you upgraded to the latest stable version?
Well that scared the daylights out of me. (had over 15 tabs open with projects i was working on) and decided to run a maximized test, and Firefox disappeared completely.
Steps I tried to get it back...
1. click on the upper part of monitor 2 and lower part of monitor 4 to get it back.. nothing.
2. moved the taskabr on monitor 4 to the left side, then tried clicking everywhere between the top of monitor 2, and lower part of monitor 4..nothing.
3. i then looked at the firefox icon in all taskabars from all 4 monitors, and none of them showed that firefox was running.
4. i then notice a hairline line going across monitor 4, so i tried clicking, dragging, double clicking, right clicking...nothing
5. i then opened task manager (applications) noticed firefox was indeed still running, i then right clicked it, and chose Switch to, Bring to front, Minimize, Maximize...Nothing, then i closed task manager, reopened it, and tried again starting with Bring to front, and instantly the firefox icon lit up again on monitor 2, and the maximized window reappeared on monitor 2.
*I always use firefox on monitor 2 and never use it maximized due to the gadgets i always have running, so never noticed this other issue you experienced until today.
*I'm on most recent version.

Another thing to look: the relation between roll up and multi monitor positions (I have 3, but all side-by-side).
this was a scary test as well...
1. if moving firefox to monitor 1 (bottom monitor in restore down state) it behaves the same as monitor 2, getting stuck between monitor 1, and monitor 3, and the only way to get it back if in restore down state is by clicking the thin line and dragging down as shown in video, if maximized same loop of issues as mentioned above.
2. if moving firefox to top monitors (3 & 4 in restore down state) Firefox title bar jumps above the top 2 monitors onto my wall (off screen) but you can however see a hairline of the bottom of the firefox window, if you then drag down and carefully position your mouse at the very top of monitor 3,4 you can get lucky enough to get your mouse pointer at the very bottom of the title bar, to get your window back. (if in maximized state one must do the loop of troubleshoots mentioned above) .

Do you have any vertical disposion?
not sure what you are asking, (sorry.) As shown in video if in restored state upon clicking the rollup window to titlebar horizontally the window/titlebar stays as set. vertically it does roll up as shown in video completely, and as expected (no weird issues as with skype) I am however noticing (and this maybe the cause/ or related to the issue) that in firefox window ONLY Everytime you click the rollup window to tilebar bar nothing happens (You must click it twice) The time frame between clicking it twice does not matter, so for example if i click the button once, nothing happens, and then wait, 10 seconds - 10 minutes the second time i click that button it will then roll up, however it will cause the issues mentioned. once i get the window back manually, then in order to roll up that window again, one must click the rollup button two times in order for it to roll up. No other windows/apps/programs seem to be doing this, they all roll up instantly and beautifully with one click (except for skype as mentioned already)
Hope this sheds some light
Thanks Guys
P.S. in hindsight, i should have went to firefox icon, and right clicked and chose to open new private window, and ran the tests with that. (Doh
