I also have this issue, and I have independently tried all of the suggestions listed here before finding this thread.
If required, I can provide any debugging logs required.
I also noticed the discrepancy on the name of the application via the "drag the target method". It has a really weird path when you do that:
C:\Program Files (x86)\DisplayFusion\Volume{f157ac14-0ef9-e0e0-a976-4e51c7d5a342}\ForzaHorizon5.exe
When you scroll down the application list to "Windows Universal Applications", you get this:
C:\Program Files\WindowsApps\Microsoft.624F8B84B80_3.410.860.0_x64__8wekyb3d8bbwe\ForzaHorizon5.exe
Note the version number there -- that will change with every patch of the game.
I can also confirm that Wildcards do not work at all:
does nothing to help this issue.

I believe your momentary success here was because you may not have waited long enough for DisplayFusion to "reinitialize" after the configuration changes before launching the game, and it just "snuck through" while DF was setting itself up again.
