Why do some programs create a new taskbar icon even though I have that program pinned?

See attached screenshot for what I mean. It's mostly Opera. I will pin Opera to the DF taskbar, then a short while later the pinned icon stops being used and a new, unpinned icon shows up in it's place. However the pinned icon is still there.
Are there any fixes for this?

I've tested this here and it seems to work ok for me. Does this seem to happen after Opera updates to a new version?

Sorry for the late response, but yes I did notice that. It seemed to only happen after certain programs updated to a new version.
